Your treatment consent

1. Procedure and expectations. The team has explained laser hair removal, its expected benefits, limitations and alternatives, including no treatment. Results vary and multiple sessions are generally needed; no outcome is guaranteed.
2. Student treatment and supervision. I understand that students perform GCI model treatments under appropriate qualified professional supervision. The team identifies the person treating me and the supervising professional.
3. Consultation and screening. My suitability and relevant history must be reviewed through the approved clinical process before treatment. I have had the opportunity to ask questions and receive answers I understand.
4. Agreed areas and price. Only the treatment areas and boundaries agreed with the team are authorized. The total session price and any booking terms are confirmed before treatment.
5. Risks and precautions. Possible effects and risks include discomfort, redness, swelling, burns, blisters, pigment changes, scarring, infection, eye injury and incomplete hair reduction. Individual and device-specific risks are discussed with me.
6. Required photography permission. Photographs of agreed treatment areas are part of model participation for documentation, student education and GCI program promotion. Faces are excluded. I must sign the separate photography waiver; this treatment form alone does not replace that permission.
7. Preparation, protection and aftercare. I receive individualized instructions, use the protective eyewear and other measures specified by the team, and know how to contact the team about concerns or unexpected reactions.
8. My right to ask questions or stop. I can ask questions, decline an area or ask for treatment to stop. Any payment, rescheduling or cancellation terms are explained separately.
9. Information and records. My completed consent and clinical information are handled through the team’s approved record process. Photography uses and privacy considerations are described in the separate waiver. No medical history or medication details belong in the public inquiry form.
10. Voluntary acknowledgment. I have reviewed the information and the specific treatment plan with the team. I understand that both treatment consent and the required photography waiver must be completed before a model session can proceed.
