Medical Outreach


This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on your income. You will be required to prove financial need in order to receive free services or services at a reduced cost. If you are uninsured, fees for service will be based on a sliding fee scale determined by income and family size according to the annual Federal Poverty Guidelines.

On the first visit, patients who are uninsured meet with an Specialist and undergo a simple income screening process. This screening identifies whether or not you or a member of your family may be eligible for assistance and also determines your fee for services based on the sliding fee scale.

Episodic care is delivered by students of GCI and it’s affiliates.
